What is social prescribing? 

Your doctor isn't the only person at your GP practice who can help you feel better. You can improve your health and wellbeing through social prescription.

Workers are based in GP practices and are part of the NHS, we are here to help you to connect with community activities, groups and services to support the needs that affect your wellbeing.

 

How do I access the service? 

It's simple, you can ask your GP, health care professional or reception staff to refer you to the social prescribing link worker (SPLW) at your GP practice.

The social prescribing link worker will then contact you and arrange either a telephone, video or face-to-face appointment with you.

Social prescribing is available to anyone who is 18 and over. Newham is working towards introducing a bespoke children's and young people's social prescribing service.

Our social prescribing link workers can help you with a range of issues, such as:

  • Social isolation and loneliness 
  • Weight management and exercise
  • Elderly support
  • Substance misuse 
  • Housing, benefits or financial issues 
  • Accessing work or work related issues 
  • Training and volunteering

  • Living with long-term health conditions 
  • Emotional wellbeing 
  • Domestic abuse
  • Community activities
